Understanding Scar Revision Surgery

Scar revision surgery is a type of plastic surgery that is used to improve the appearance of scars. It can involve removing, reshaping, or recontouring the scar tissue. Scar revision surgery may also involve the use of skin grafts or other surgical procedures to improve the appearance of the scar.

There are multiple options for improving the appearance of facial scars, and they vary widely from minimally invasive treatments that refine the skin tone and texture to advanced plastic surgery that will restore a more natural look.

Factors that can affect scar revision

The right treatment approach for a facial scar and the overall improvement in appearance depends on individual factors that include:

  • The location and extent of scarring
  • The type of scarring (such as from acne, a burn, an injury, or prior surgery)
  • Skin tone, texture, and color
  • Age
  • Personal healing rate

Different types of scarring

Scars are a byproduct of the body’s healing process, and in some cases, the effects of healing include keloid scars, hypertrophic scars, or contracture scars.

  • Keloid scars are caused by the body’s overproduction of collagen at an injury site after the skin heals, and they often appear in red, growth-like clusters in the scar site. 
  • Contracture scars are pucker-like marks that often accompany burns; they can also limit muscle and tendon function.
  • Hypertrophic scars are typically red, thick scars that appear within an injury or incision site; they may restrict the movement of underlying muscles and tendons.

Non-Surgical scar revision options

If scarring is minor and the surrounding skin is otherwise healthy and has consistent texture and coloring, procedures such as dermabrasion or a chemical peel may be used to soften the rough texture and rejuvenate skin tone.

Minor concave or depressed scars may respond to dermal fillers designed to plump the skin and restore a smooth tone. In some cases, laser resurfacing can improve the appearance of the skin tone and texture and stimulate the production of new skin cells at the scar site.

When scarring is more severe or affects muscle movements and facial expressions, surgery may be recommended.

About facial scar surgery

Several surgical techniques can reduce the visibility of facial scars. Surgery is generally intended to minimize the appearance of the scar and make the scar tissue look consistent with the surrounding skin tone, texture, and color.

  • Plastic surgery can remove some contracture scars entirely, leaving a smooth, virtually seamless appearance. 
  • In some circumstances, the scar itself may be repositioned with a technique often referred to as a Z-plasty. This procedure shifts the scar to follow natural facial contours, which makes the scar less conspicuous. 
  • In other cases, flaps of adjacent healthy skin may be lifted and repositioned to improve both appearance and functionality.
  • If scarring is severe, skin grafts or multiple treatments may be necessary to achieve optimal results.
